I also had an initial reaction that I’d be able to find several such dramas if I browsed through my list on MDL, but I was surprised to find this wasn’t the case. This seems like a more common trope among C-dramas, based on what I have watched anyway. And to be fair, for dramas I’ve watched much further back, I was unable to remember what the SML’s role was.
In the end, I didn’t come up with a single one that met all the criteria. This one meets most, but the male childhood friend with the crush is not really the second male lead; he’s more like the third male, but he plays an important role in the FL’s life:
